Glacier Medical Associates Health Screen Proposal

• 12-Week Wellness Contract
• Phase I - Health Screening
  • Blood Pressure
  • Lipid Panel - Cholesterol, LDL, HDL, VLDL, Trig
  • Fasting Blood Sugar
• Target Date - 12 weeks
• Lab and Bp Results Reviewed by Cathy Relf, RN/MS
  • Follow-up presentations and referral of high risk employees.
• Phase II - Weekly intervention by Cathy Relf, RN/MS
  • Four (4) hours/week to meet with employees to set goals and help individuals achieve lifestyle. changes- decrease risk and improve health.
  • Nurse available on-site for 12 weeks
• Phase III -  Completion of 12-Week Wellness Program
  • Rescreening for high risk employees
  • Statistical analysis of results
    • % decrease in weight, blood pressure, fasting blood sugar, cholesterol results.
    • % increase in exercise
    • % increase in employees stopping smoking, etc.
• Referrals
  • Employees will be referred to primary care physician for preventative health care and follow up.
