Vaccines protect not only yourself but entire communities. That is why vaccines are vital to the public health goal of preventing diseases. If you need a copy of your immunization records, you may log in to the Patient Portal to print any immunizations you have received at Glacier Medical Associates.

  • Birth or 1 month = #1 Hep B
    2 months = #1 DTaP / #1 Polio / #1 HIB / #1 PCV / #1 ROTA
    4 months = #2 DTaP / #2 Polio / #2 HIB / #2 PCV / #2 ROTA
    6 months = #3 Hep B / #3 DTap / #3 HIB / #3 PCV / #3 ROTA
    12-15 months = #4 HIB / #4 PCV / #1 MMR / #1 VARI
    15-18 months = #4 DTaP / #3 Polio / #2 VARI / #1 Hep A
    18-24 months = #2 Hep A
    4-6 years = #6 DTaP / #4 Polio / #2 MMR
    11-19 years = #1 TD/TDAP* / #1 HPV / #1 MEN
    * TD = Tetanus/Diphtheria to be given every 10 years
